All patients must complete an Appointment Request Form to begin the scheduling process.

Once this form is completed, you will receive by email a new patient packet to fill out and return before scheduling an appointment.

Evaluation: You will be completing a computerized interactive testing that measures the three core symptoms of ADHD, hyperactivity, impulsivity, and inattention, and answering an individualized questionnaire. 

Consultation: You will meet with one of our providers to discuss your medical history, symptoms and concerns, and results of your testing.

Treatment Plan: We will work with you to develop a personalized treatment plan tailored to your specific needs.

To help us provide the best care possible, please bring the following items to your appointment:

• Insurance card
• Photo ID
• List of current medications
• Method of Payment. Payment is collected at the time of service.
